Form 1095-C is an information document used to help administer certain provisions of the Affordable Care Act (ACA). Per the Internal Revenue Service (IRS), the information on Form 1095-C is not needed to file your income tax returns with the IRS.

2/90 Sign Systems is required to provide Form 1095-C, which outlines your health insurance coverage for the previous year. Due to the recently enacted Paperwork Burden Reduction Act, employers are no longer required to automatically distribute Form 1095-C to all employees.

Form 1095-C is now available upon request.
